Losing Temple Limbs while in console PS5 couch coop.

My wife and I play on PS5 couch coop all day. Her character keeps losing her temple limbs when she is NOT dying. She does not want to coop any more because she lost BIS legs and arms twice with no deaths. My character has not lost limbs. can you help to get my wife to play with me again? Love the game but still very glitchy.
